Prostate tumor cells and supports transendothelial migration through BMECs 33 ; , indicating its putative role in the migration of circulating metastatic prostate tumor cells into bone. However, understanding the molecular mechanism by which tumor and endothelial cells initiate contact in blood flow may also offer new insight into the organotropism of tumor metastasis and perhaps reveal novel therapeutic targets for antimetastatic drug development. In this report, we show heretofore unrecognized E-selectin ligand activities on human metastatic prostate tumor cells. By using human BMECs to examine the expression of E-selectin ligand activities, we determined whether other known adhesive mechanisms responsible for preferential adhesion of prostate tumor cells to BMECs could promote cell attachment or rolling behavior under shear flow. Under physiological flow conditions present in BM 34 ; , E-selectin ligands supported prostate tumor cell tethering and rolling on E-selectin expressed on BMECs. Most notably, the bone-metastatic prostate tumor MDA PCa 2b cell line expressed robust E-selectin ligand activity, which was found to be attributable to both sialyl Lewis X-bearing glycoprotein s ; and glycosphingolipids. PC-3M LN-4 cells, originally derived from bone metastases PC-3 cells ; , also displayed E-selectin ligand activity, although the major E-selectin-binding determinants appeared to be exclusively on sialylated glycosphingolipids. Furthermore, although these binding interactions were dependent on terminal sialic acids, expression of sialyl Lewis X itself did not correlate directly with the capacity of tumor cells to bind E-selectin as suggested previously 35 ; . The market share for PPIs -- Prilosec omeprazole ; and Prevacid lansoprazole ; and Protonix pantoprazole ; , which was introduced in 2000 -- grew from 46.4 percent in 1998 to 55.5 percent in 1999 to 60.3 percent in 2000. In 2000 the AWP per prescription costs for Prilosec and Prevacid grew about 7 percent to $148.96 and $134.23, respectively, compared to $88.09 for ranitidine. Along with an 8.5 percent increase in utilization for this class, the shift to the use of more expensive PPIs contributed an over $6 PMPY cost increase in this class. Approval for Nexium esomeprazole ; , was granted by the FDA in February 2001. Currently the only second generation PPI available in the United States, Nexium has indications for relieving GERD symptoms, for healing erosive esophagitis, and for eliminating H. pylori when taken in combination with amoxicillin and Biaxin clarithromycin ; . It is marketed by the same company that sells Prilosec . Patent protection for Prilosec, originally scheduled to expire as early as April 2001, will be extended for six months while the manufacturer conducts studies in pediatric patients. Release of a generic has been further delayed by lawsuits against potential generic suppliers. A request for OTC status for Prilosec was denied by the FDA and estrace.

Hearing loss is not a frequent symptom of MS. About six percent of people who have MS complain of impaired hearing. Deafness due to MS is exceedingly rare, and most acute episodes of hearing loss caused by MS tend to improve. Because hearing deficits are so uncommon in MS, people with MS who do develop hearing loss should have their hearing thoroughly evaluated to rule out other causes. Reduced ability to communicate effectively can have a serious impact on social interactions. The individual may be embarrassed, communicating may become a very tiring effort, and the individual may discontinue trying to engage socially. Home healthcare providers must be patient when communicating with an individual with hearing loss and not pretend they have understood when they have not. Speech language therapy may be effective in assessing for hearing aids or other listening devices and in recommending other strategies to compensate for hearing loss.
